RN Hospice Field Visits

About the role

Trustbridge Hospice, part of Empath Health, is seeking a compassionate and motivated Registered Nurse (RN) to provide emergent hospice field visits throughout Palm Beach County. Since 1978, Trustbridge Hospice has been honored to serve families across Palm Beach and Broward counties with compassionate, community-based care.

Responsibilities

• Provide rapid-response, patient-centered clinical care to individuals in home and community settings.
• Assess, triage, and manage urgent and emergent health conditions outside of traditional clinical environments.
• Operate with a high level of autonomy while collaborating with physicians, advanced practice providers, and interdisciplinary teams to deliver timely, high-quality care and prevent unnecessary emergency department visits or hospitalizations.
• Serve as a key advocate for patients and families, ensuring their needs are recognized and addressed.
• Coordinate comprehensive care under the direction of the primary physician and in collaboration with the interdisciplinary team.
• Use clinical expertise to monitor patient conditions, provide interventions, and deliver education that enhances comfort and quality of life.

Requirements

• Registered Nurse currently licensed in the State of Florida.
• Current Basic Life Support Certification.
• One year in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing or Hospice nursing preferred.
• Understanding of and ability to apply characteristics of Hospice Nursing: case management, crisis intervention, communication skills, teaching skills, thanatology skills and needs.
• Basic computer skills and EMR software experience a plus.

• Adequate and reliable means of transportation with adequate insurance and valid Florida driver’s license.
