Are you a nurse looking for a change? At Hackensack Meridian Health At Home, our nurses focus on one patient at a time, spending valuable and meaningful 1:1 time with patients in their homes. This role offers a flexible schedule in a setting with fewer distractions, allowing you to do what you do best.

Responsibilities

  • Provide professional nursing care to patients in their home through assessment, development, implementation, and evaluation of home nursing care needs.
  • Ensure compliance with physician orders, including visit frequency and timely documentation reflecting progression to goals.
  • Demonstrate holistic patient care, ensuring safe and effective outcomes through clinical judgments and critical thinking.
  • Complete assigned treatments and procedures according to established standards and policies.
  • Utilize the interdisciplinary team and document all activities accurately.
  • Identify appropriate payors and comply with authorizations for self and other disciplines.
  • Maintain visit frequency within authorization range and use secure messaging for insurance communications.
  • Complete nursing assessments, medication reconciliation, and collaborate with physicians on the Plan of Care.
  • Update care plans within two weeks of changes in patient status and implement teaching strategies consistent with goals.
  • Report patient care problems to management or physicians in a timely manner with possible solutions.
  • Act as a liaison among physicians, managers, and healthcare team members to coordinate services effectively.
  • Communicate pertinent patient information and Plan of Care during transfers or time-off.
  • Complete routine assignments accurately, consistently, and independently while meeting productivity requirements.
  • Act as a clinical supervisor for para-professional home health aide staff, completing supervisory visits within regulatory time frames.
  • Participate in the evaluation of home health aide staff.
  • Maintain awareness of updated standards and incorporate them into nursing practice.
  • Demonstrate flexibility with job responsibilities and adhere to organizational competencies and standards of behavior.
  • Perform physical tasks such as lifting a minimum of 50 lbs., pushing/pulling 50 lbs., and standing for a minimum of 8 hours a day.
  • Other duties and/or projects as assigned.

Qualifications

  • Associate's Degree in Nursing or Diploma Certificate in Nursing.
  • At least six (6) months of RN experience or at least 1 year of LPN experience, preferably in home care or hospice.
  • NJ State Professional Registered Nurse License.
  • AHA Basic Health Care Life Support HCP Certification.

Preferred: BSN.

Pay

Minimum annual base pay of $90,272.00. The starting rate may vary based on factors such as labor market data, experience, education, certifications, skills, geographic location, internal equity, and budget considerations. Certain roles may also be eligible for shift differentials, performance-based incentives, bonuses, or commissions.
