RN-Hospice

This role operates on an 8 AM–5 PM Monday–Friday schedule with no weekends or after-hours call.

About the Role

Provide nursing care for hospice patients in the Tri-County area (specific zip-code coverage to be determined). The Registered Nurse (RN) plans, organizes, and directs hospice care, supervises and coordinates care for patients with terminal illnesses, and delivers comprehensive skilled nursing services in patients’ homes or the hospice inpatient setting to meet the agency’s goals.

Responsibilities

• Deliver direct hands-on care in accordance with the patient’s plan of care and South Carolina Nurse Practice Act standards.
• Assess, plan, implement, and evaluate patient care; administer medications and manage symptoms and end-of-life needs.
• Communicate with physicians regarding patient needs, report condition changes, and obtain orders as required.
• Teach self-care techniques, arrange for necessary equipment and services, and maintain a safe patient environment.
• Complete, maintain, and submit accurate clinical notes and documentation.
• Attend interdisciplinary group meetings and participate in quality assessment and performance improvement activities.
• Supervise care provided by LPNs and Hospice Aides to ensure treatment goals are met; delegate responsibilities as needed.
• Coordinate with other disciplines, agencies, families, and companies to meet patient care needs.
• Participate in on-call duties as defined by the on-call policy.

Requirements

• Current South Carolina RN license or a compact/multi-state RN license from a recognized NCSBN Compact State (not a permanent SC resident).
• Current CPR certification.
• Valid South Carolina driver’s license, good driving record, and reliable transportation for home visits.

Work Environment

• Frequent standing, walking, bending, crouching, stooping, and driving.
• Frequent lifting/moving of patients; intermittent lifting, moving, or carrying up to 50 lbs.
• Frequent pushing or pulling over 50 lbs.
• Constant use of finger/hand dexterity, eye/hand coordination, and reaching with hands/arms.
• Frequent talking or hearing; corrected hearing and vision to normal range.
• Visual requirements: patient inspection, assessment, gauge visualization, and documentation.
• Exposure to blood, body fluids, communicable diseases, unclean environments, toxic substances, and hazardous driving conditions.
• Intermittent risk of exposure to blood-borne pathogens.
• Moderate noise levels; work under stressful conditions, deadlines, or irregular hours.
• Occasional on-call evenings and weekends based on department needs.
