RN - Hospice

13-week contract assignment supporting a leading healthcare organization in Menominee, MI.

Responsibilities

• Coordinate end-of-life care for hospice patients by serving as the primary liaison between patients, families, physicians, the hospice medical director, and the interdisciplinary care team.
• Conduct comprehensive physical, psychosocial, and spiritual assessments while developing and managing individualized care plans for patients in their homes or residential facilities.
• Manage pain and symptoms, educate patients and caregivers on disease progression, medication management, and the grieving process, and provide compassionate support throughout the hospice journey.
• Manage a daily caseload of 5-6 patient visits while ensuring high-quality, patient-centered care.

Requirements

• Active Michigan Registered Nurse (RN) license.
• Current CPR certification.
• Prior hospice nursing experience preferred.
• Strong assessment, care coordination, communication, and patient education skills.

Schedule

• 13-week contract starting September 14, 2026.
• Monday-Friday, 8:00 AM - 4:30 PM (8-hour shifts / 40 hours per week).
• On-call requirement: every other weekend and 2-3 weekdays per week.
• Productivity expectation: 5-6 patient visits per day.
• Coverage area: Menominee County, MI.

Pay

• Pay range: $53.00-$57.00/hour.
• Mileage reimbursement from the first patient visit to the last patient visit of the day.
