Seeking an experienced and compassionate Registered Nurse (RN) Case Manager to join its team in Huntingdon, Tennessee. This position provides skilled hospice nursing care to patients facing life-limiting illnesses while supporting their families through education, care coordination, and symptom management.
Schedule
- Monday through Friday, 8:00 AM to 4:30 PM
- 40 hours per week
- Shared rotation for weeknight and weekend on-call coverage among all case managers
- Mileage reimbursement provided between patient visits
Pay
$51.00 - $53.00 per hour
Responsibilities
- Serve as the primary RN Case Manager for an assigned hospice patient caseload
- Provide skilled nursing assessments, symptom management, and end-of-life care in accordance with hospice standards and physician orders
- Develop, implement, and update individualized plans of care based on patient needs and goals
- Coordinate care with physicians, hospice aides, social workers, chaplains, and other interdisciplinary team members
- Monitor patient conditions and communicate changes promptly to the appropriate members of the care team
- Provide education and emotional support to patients and families regarding disease progression, medications, symptom management, and hospice services
- Maintain accurate, timely, and compliant clinical documentation
- Participate in interdisciplinary team meetings and care planning discussions
- Support agency operations during periods of census growth and increased patient needs
- Participate in the agency's rotating on-call schedule, including weekends and evenings as assigned
- Promote patient comfort, dignity, quality of life, and compassionate end-of-life care
Requirements
- Active Registered Nurse (RN) license
- Previous hospice case management experience required
- Strong clinical assessment and critical thinking skills
- Excellent communication and interpersonal abilities
- Valid driver's license, reliable transportation, and proof of automobile insurance
- Ability to work independently while effectively managing a field-based caseload

- Experience working within an interdisciplinary hospice team
Dress Code
Black or Royal Blue scrubs preferred.
