We are seeking an experienced and compassionate Hospice Registered Nurse (RN) to provide skilled end-of-life care to patients throughout the New Orleans Metroplex. The ideal candidate will manage a patient caseload, coordinate care with the interdisciplinary team, and support patients and families with high-quality hospice services.
Schedule & Pay
- Location: New Orleans Metroplex
- Schedule: Monday through Friday, 8:00 AM to 4:30 PM
- Hours: 8 hours per day
- Pay Rate: $51.00 - $53.00 per hour
- Productivity Expectation: Minimum of 25 visits per week
- On-Call Requirement: Participation in rotating on-call schedule required
- Mileage Reimbursement: Reimbursed between patient visits; commuter miles are not eligible for reimbursement
Responsibilities
- Manage and direct the care of assigned hospice patients in accordance with physician orders, hospice regulations, and agency policies.
- Perform comprehensive nursing assessments and ongoing evaluations of patient status and care needs.
- Maintain accurate, timely, and compliant clinical documentation, including adherence to company documentation and synchronization requirements.
- Complete medication reconciliation during every patient visit and coordinate medication ordering and acquisition as prescribed by the physician.
- Develop, implement, and update individualized plans of care in collaboration with the interdisciplinary care team.
- Supervise and evaluate home health aides, ensuring assigned duties are performed appropriately and patient care goals are achieved.
- Complete required supervisory aide visits and document findings accurately.
- Follow hospice admission, recertification, and regulatory processes, reviewing findings with clinical leadership as required.
- Participate in Interdisciplinary Group (IDG) meetings and ensure all IDG documentation is completed prior to meetings.
- Collaborate with physicians, social workers, chaplains, aides, and other team members to provide coordinated patient-centered care.
- Attend daily morning huddles, nursing meetings, staff meetings, and other required agency activities.
- Provide patient and family education related to disease progression, symptom management, medications, and hospice services.
- Participate in the agency's on-call rotation to ensure continuity of patient care.
Requirements
- Active Registered Nurse (RN) license in the applicable state.
- Prior hospice nursing experience required.
- Proficiency with HCHB (Homecare Homebase/PointCare) documentation platform.
- Strong knowledge of hospice regulations, documentation requirements, and patient care practices.
- Valid driver's license, reliable transportation, and current automobile insurance.
- Excellent assessment, clinical judgment, communication, and organizational skills.
- Ability to work independently while managing a field-based caseload.
Preferred Qualifications
- Experience caring for General Inpatient (GIP) hospice patients.
- Experience managing complex patient populations, including:
- Cardiac drip therapies
- Airvo high-flow therapy patients
- Tracheostomy care
- Other high-acuity hospice patients
- Strong interdisciplinary collaboration skills.
- Experience mentoring aides or participating in staff education and training.
Dress Code
No specific dress code required. Preferred scrubs: Navy or Gray.
