RN Hospice After Hours PRN

About the Role

The Registered Nurse (RN) is responsible for assessing, planning, implementing, coordinating, monitoring, and evaluating the strategies and services required to meet the patients' and families' end-of-life care needs. An outcome-based plan of care is created and continually adjusted using the nursing process, relationship-centered care principles, and a holistic philosophy. This ensures the changing needs of the patient and family are met while facilitating a pain-free, peaceful, and sacred death.

Responsibilities

• Exhibit a "Patients and Families First" attitude, responding with compassion, flexibility, and urgency to each patient’s needs.
• Provide care that patients want when they want it.
• Develop a comprehensive plan of care addressing physical, emotional, cultural, and spiritual needs of the patient and family, with input from the patient, family, and interdisciplinary team.
• Evaluate the plan of care and make interventions as necessary.
• Advocate for pain and symptom management.
• Educate and provide support to the patient and family regarding care issues.
• Commit to continuous improvement of the patient experience, including patient safety, quality of care, and excellent service.
• Adhere to the organization’s Mission, Service Standards, and Code of Conduct.
• Direct and supervise LPNs and CNAs.
• Administer and document medications and treatments as ordered by the physician according to policies in an accurate and timely manner.
• Identify and respond appropriately to emergency situations.
• Attend shift reporting and Interdisciplinary Team (IDT) meetings to facilitate communication of decisions related to patient care.
• Actively participate in safety initiatives and risk-mitigating measures; complete all position and unit safety-related competencies and requirements on time.
• Perform other duties as assigned.

Requirements

• Associate’s degree in nursing (required).
• 1 year of relevant experience, preferably in Hospice (required).
• Michigan Registered Nurse (RN) license upon hire (required).
• CPR certification (American Heart Association) within 90 days of hire (required).
• Bachelor’s degree in nursing (preferred).

Schedule

Part-time, night shift.

• Weekly scheduled hours: 8
• Hours of work: 4 p.m. to 1:30 a.m. or 9 p.m. to 9:30 a.m.
• Days worked: Variable
• Weekend frequency: Variable weekends
