About the role
As an After Hours/On Call Registered Nurse, you’ll contribute to our success by ensuring high-quality hospice care for patients in their preferred home setting. You will assess patient needs, communicate findings, and support patients, families, and caregivers through compassionate, interdisciplinary care.
Responsibilities
- Ensure all activities align with the vision of Elara Caring’s board of directors, executive team, and Hospice leadership.
- Assess hospice care patients and families to identify physical, psychosocial, and environmental needs, documented through clinical records, Interdisciplinary Team reports, after-hours reports, and on-site evaluations.
- Obtain data on physical, psychological, social, and spiritual factors influencing patient/family/caregiver health status.
- Communicate significant findings, problems, or changes in health condition, environment, or unsafe facility conditions to the Clinical Supervisor, physician, facility, or other involved personnel.
- Counsel, instruct, and include the patient, facility, and family in following the Interdisciplinary Plan of Care.
- Teach hospice philosophy and pain/symptom management to staff in alternate care settings (nursing homes, hospitals, assisted-living facilities, etc.).
- Provide appropriate support at time of death and perform bereavement assessment.
Requirements
- Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing.
- Current State License as a Registered Nurse (RN).
- 1 year of experience in a clinical care setting.
- Experience in a hospice or home health environment is preferred.
- 50% travel required.
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
- Valid driver’s license, insurance, and reliable transportation to perform job tasks.
