RN Home Infusion (FT, 40hrs/week) - Home Infusion Services
About the role
The RN Home Infusion position is a mobile role based in Philadelphia and its surrounding areas, including Southern NJ and Northern DE. The role involves planning, coordinating, and providing individualized nursing care to patients and families in their homes. This position reports to the Care Team Leader or Nurse Manager of the Department and interacts with patients, co-workers, and other staff.
Responsibilities
- Planner for, coordinator of, and provider of individualized nursing care to patients and families in their homes.
- Reflects the mission, philosophy, and values of the agency.
- Performs patient care in a timely and efficient manner.
- Maintains productivity expectations.
- Maintains a positive impact on customers (internal and external).
- Implements, reviews, and revises plans of treatment.
- Performs patient assessments as required by patient condition and/or by agency policy.
- Provides client education for clients/caregivers receiving IV therapy at home.
- Initiates, maintains, and discontinues IV services with clients in a complete, thorough, and timely manner.
- Sets up and discontinues IV services with clients in a complete, thorough, and timely manner.
- Submits documentation within the 48-hour guideline.
- Communicates with other members of the health care team on issues pertaining to client care needs, status updates, and coordination of care issues.
Requirements
- Graduate of an accredited nursing school.
- At least 2 years of experience in the acute care setting or 1 year in home care, hospice, or home infusion setting.
- Current RN licensure in the state of employment.
- Provider in Basic Cardiac Life Support.
- Valid driver’s license.
