About the Role

This position involves providing specialized nursing services in a home care setting, including patient assessments, developing and implementing individualized care plans, and coordinating care with an interprofessional team. The nurse evaluates patient needs, respects cultural and emotional considerations, and ensures quality, evidence-based care to promote patient safety and outcomes.

Responsibilities

• Assess patient preferences, barriers, and diverse needs
• Develop, document, and communicate personalized care plans
• Collaborate with team members and family/caregivers
• Evaluate patient condition changes and update care accordingly
• Ensure safe, timely, and evidence-based care delivery

Requirements

• RN license with a nursing diploma or associate's degree (bachelor’s preferred)
• Less than 2 years of experience; 2-5 years preferred
• Valid driver's license and ability to travel locally
• Strong communication, critical thinking, and cultural competency skills
• Ability to work in a safety-sensitive environment

Schedule

Part-time role (24 hours/week) with flexible day shifts. Rotation required for weekends, holidays, and on-call duties.

Additional Information

Regular local travel within the service area, with mileage reimbursement in line with IRS guidelines.
