Role Overview This position involves providing specialized nursing services in a home care setting, including patient assessments, developing and implementing individualized care plans, and coordinating care with an interprofessional team. The nurse evaluates patient needs, respects cultural and emotional considerations, and ensures quality, evidence-based care to promote patient safety and outcomes. Responsibilities include monitoring patient conditions, updating care plans, and advocating for patient-centered care. Key Responsibilities Assess patient preferences, barriers, and diverse needsDevelop, document, and communicate personalized care plansCollaborate with team members and family/caregiversEvaluate patient condition changes and update care accordinglyEnsure safe, timely, and evidence-based care delivery Qualifications & Skills RN license with a nursing diploma/associate's degree (bachelor’s preferred)Less than 2 years of experience; 2-5 years preferredValid driver's license and ability to travel locallyStrong communication, critical thinking, and cultural competency skillsAbility to work in a safety-sensitive environment Additional Information Part-time role (24 hours/week) with flexible day shiftsRotation required for weekends, holidays, and on-call dutiesRegular local travel within the service area, with mileage reimbursement in line with IRS guidelines
