Role Overview

This position involves evaluating clients, developing individualized care plans, and providing specialized nursing services in a home care setting. The nurse collaborates with a multidisciplinary team, supports patient self-care, and ensures continuity of care through effective communication and documentation.

Qualifications & Skills

• Licensed RN with a valid driver’s license
• Diploma or Associate’s degree in Nursing, with preferred Bachelor’s degree in Nursing or Home Health
• Minimum of 2 years of nursing experience; no supervisory experience required
• Strong assessment, communication, and critical thinking skills

Key Duties

• Develop, implement, and document care plans in collaboration with patients and team members
• Provide culturally competent, patient-centered care supporting emotional and spiritual needs
• Evaluate patient condition changes, communicate with caregivers and team, and update care plans
• Support safe care delivery and adherence to evidence-based practices
