Responsibilities
- Provide skilled nursing care in patient homes
- Complete OASIS assessments and documentation
- Develop and implement patient care plans
- Carefully coordinate care with physicians and care teams
- Education of patients and families on care plans
- Management of caseload and meeting productivity requirements
- Support for hospice and home health services
Requirements
- Active Idaho RN License
- BLS Certification
- Home Health RN experience
- OASIS documentation experience
- Homecare Homebase (HCHB) experience
- Strong case management skills
- Maintain productivity targets (30 points/week)
- Valid driver’s license and reliable transportation (for home visits)
