BJC Home Care is seeking a Registered Nurse to provide direct patient care with work-life balance, schedule flexibility, and the opportunity to build meaningful relationships with patients in the community.
About the Role
BJC Home Care offers a complete range of home care services, including skilled nursing, hospice and supportive care, rehabilitation therapy, home infusion therapy, home medical equipment, and high-tech respiratory care. Specialty programs include asthma, cardiac, diabetes, orthopedic, and wound care. Serving over 25 counties in Missouri and Illinois, BJC Home Care is the largest home care network in the region.
The Metro St. Louis Intermittent Home Care Department provides home visits with 24-hour on-call nursing supervision, combining advanced technology with a belief in the recuperative advantages of home care. This position requires registration with the Family Care Safety Registry and coverage across all St. Louis regions during on-call shifts.
Schedule
- 40-hour work week with flexible scheduling options, including 4-day workweeks (10-hour shifts)
- Typical business hours: Monday through Friday, 8:00 AM to 4:30 PM
- Weekend rotation: generally every 4th weekend
- Paid on-call nights: 4–6 per month
- 2 holidays per year
Responsibilities
- Assess patient preferences, barriers to care, and population-specific needs (emotional, spiritual, cultural)
- Develop, implement, and document individualized care plans with defined goals in collaboration with the interprofessional team, patient, and family/caregiver
- Ensure communication of the care plan across the continuum of care
- Promote respect, equity, and empathy in interactions with diverse and vulnerable populations
- Practice collaborative problem-solving, service recovery, and advocacy for patient- and family-centered continuity of care
- Integrate interprofessional team data and critical thinking to implement care safely and timely
- Evaluate changes in patient condition, update the care plan in the EMR, and communicate with the team and family/caregivers
- Ensure evidence-based practice and quality patient outcomes
- When assigned as case manager, coordinate all aspects of patient care
This is a safety-sensitive position requiring constant alertness and safe work practices.
Requirements
- Valid Driver’s License
- RN licensure
- Willingness to provide coverage in all St. Louis regions during on-call shifts
Preferred Qualifications
- Bachelor’s Degree in Nursing or Home Health
- 2–5 years of experience in nursing/home health
