Role Overview

Responsibilities

• Assessing patient needs, preferences, and barriers to care
• Developing, implementing, and documenting personalized care plans in collaboration with patients, families, and the care team
• Promoting respect, equity, and empathy across diverse populations
• Monitoring patient condition changes, communicating updates, and updating care plans accordingly
• Ensuring evidence-based practice and quality outcomes

Qualifications & Skills

• Valid RN license and driver’s license
• Diploma or Associate’s degree in nursing; Bachelor’s preferred
• Less than 2 years of experience (2-5 years preferred)
• Ability to work safely and be alert at all times
