RN Home Health - PRN

About the role

Seeking a PRN Home Health Registered Nurse to deliver skilled nursing care through home visits, assessing patient needs, developing and implementing individualized care plans, and collaborating with interprofessional teams. The role involves evaluating patient preferences, cultural and emotional needs, and providing education to promote self-care. The nurse will monitor patient conditions, communicate updates, and ensure evidence-based practices are followed to achieve quality outcomes.

Responsibilities

• Assessing patient barriers and care preferences
• Developing, implementing, and documenting personalized care plans
• Promoting respectful, equitable, and culturally sensitive care
• Collaborating with patients, families, and team members
• Evaluating patient condition changes and updating care plans accordingly
• Ensuring safety and adherence to clinical standards

Qualifications

• Associate's Degree in Nursing or higher
• Valid RN license and driver's license
• Minimum of 2 years nursing experience; 2-5 years preferred
• Experience in home health or related fields is advantageous
