RN Home Health - PRN

About the role

This position involves providing skilled nursing care in a home health setting, evaluating patient needs, developing individualized care plans, and collaborating with an interprofessional team to ensure quality outcomes. The RN assesses patient preferences, cultural, emotional, and spiritual needs, and communicates care plans effectively across the continuum of care.

Qualifications

Required:

• Nursing Diploma or Associate's Degree
• RN licensure
• Valid driver’s license
• At least 2 years of experience

Preferred:

• Bachelor’s degree
• 2-5 years of home health experience

Schedule

This role requires flexibility to work two full shifts within a six-week period, with typical hours Monday through Friday, and includes holiday rotation.
