RN – Home Health Nurse | Weekend Support | PT/PRN

About the role

A home health agency is seeking a Registered Nurse (RN) with experience in caring for patients with tracheostomies and ventilators. The ideal candidate will provide weekend support and work part-time or per diem.

Responsibilities

• Assess patient needs and develop care plans
• Administer medications and treatments as prescribed
• Monitor patient condition and report changes to healthcare providers
• Collaborate with interdisciplinary team to coordinate care
• Document patient progress and communicate with family members

Requirements

• Bachelor’s degree in Nursing from an accredited program
• Current RN license in good standing
• Experience in acute care settings preferred
• Tracheostomy and ventilator care experience required
• Weekend availability
