About the role

Provides patient care in the home setting under the supervision of a licensed nurse.

Responsibilities

• Assesses patients' health status and develops care plans accordingly.
• Administers medications and treatments as prescribed.
• Monitors patients' progress and reports changes to supervising nurse.
• Ensures compliance with healthcare policies and procedures.
• Communicates effectively with patients, families, and other healthcare providers.

Requirements

• HCHB, Oasis, and HH experience required.
• OASIS Experience - Required.
• Current state license as a Registered Nurse - Required.
• CHPN Certification - Preferred.
• Proof of current CPR certification - Required.
• Current driver's license - Required.
• Must have reliable transportation and insurance - Required.
• 1-2 years experience as a Registered Nurse in a clinical care setting - Required.
• 2+ years Home health experience - Required.

Qualifications

• Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program - Required.
• Bachelor's of Science in Nursing - Preferred.
