RN - Home Health - Full Time

About the role

Provides home-based patient care including case management, assessments, discharges, patient teaching, dressing changes, IV therapy, and general skilled nursing following policy and procedures. The role involves ensuring patient safety and assisting with community resources, documentation via Epic system, and performing visits as needed in a Per Diem capacity.

Key Responsibilities

• Deliver high-quality, patient-centered care in patients' homes.
• Manage multiple patients with varying acuity levels, coordinate admissions, and discharge planning.
• Perform clinical duties such as dressing changes, IV therapy, and access site care.
• Document care accurately and timely using electronic health records.
• Assist with patient education and connecting patients to community programs.

Qualifications & Skills

• Graduation from an accredited nursing program and current RN licensure in Missouri.
• Preferably one year of experience in home health or acute care.
• Valid driver's license, auto insurance, and BLS certification through the American Heart Association.
• Strong clinical skills, compassion, and ability to work independently.
