About the role

This position involves providing specialized nursing services through home visits, assessing patient needs, developing individualized care plans, and collaborating with an interprofessional team to ensure quality, patient-centered care. The nurse evaluates patient preferences, addresses barriers to involvement, and promotes respect and empathy across diverse populations. The role typically supports the Sullivan, MO area and demands a collaborative approach, critical thinking, and adherence to safety standards.

Responsibilities

• Assessing patient needs and preferences, considering emotional, spiritual, cultural, and social factors.
• Creating, implementing, and documenting tailored care plans with clear goals, ensuring effective communication across care teams.
• Providing culturally competent care, supporting patient self-care, and advocating for continuity of care.
• Monitoring patient condition changes, updating care plans, and collaborating with families and team members.
• Ensuring evidence-based practices to achieve optimal outcomes.

Qualifications & Skills

• Nursing diploma or associate degree in nursing (RN license required).
• Minimum of 2 years of nursing experience; preferred Bachelor’s in Nursing or related home health experience.
• Valid driver’s license and ability to work safely and alertly.
• Experience in case management and working with diverse populations is advantageous.
