Key Responsibilities
- Provide skilled nursing care in patients' homes.
- Manage a caseload of home health patients and coordinate care plans.
- Complete comprehensive patient assessments and reassessments.
- Accurately document care utilizing OASIS and agency documentation standards.
- Collaborate with physicians, therapists, caregivers, and interdisciplinary teams.
- Educate patients and families regarding disease management, medications, and treatment plans.
- Monitor patient progress and adjust care plans as needed.
- Ensure compliance with agency, state, and federal regulations.
Required Qualifications
- Active and unrestricted Registered Nurse (RN) license in North Carolina.
- Previous skilled home health experience.
- Proficiency in OASIS documentation.
- Valid driver's license and reliable transportation.
- Strong clinical assessment, communication, and case management skills.
Preferred Qualifications
Candidates with the following experience will stand out:
- Wound care management.
- IV therapy administration.
- PICC line care and lab draws.
- Venipuncture and specimen collection.
- Strong home health case management experience.
