About the role
Provides direct patient care in a home health setting.
Responsibilities
- Administers medications and treatments as prescribed
- Assesses patient’s condition and documents findings
- Collaborates with interdisciplinary team to develop and implement care plans
- Monitors patient progress and reports changes to healthcare providers
- Ensures patient safety and comfort
Requirements
- HCHB, Oasis, and HH experience required
- Current state license as a Registered Nurse
- CHPN Certification preferred
- Proof of current CPR certification
- Current driver's license with insurance
- One to two years experience as a Registered Nurse in a clinical care setting
- Two plus years Home health experience
Qualifications
- Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program
- Bachelor's of Science in Nursing preferred

$15.00 per hour
Schedule
Monday through Friday, 8:00 AM - 5:00 PM, 40 hours per week. On-call required if requested.
