Travel contract / Local Contract (13‑week contract)

Pay

$52 to $58 per hour on W‑2 (based on experience, expertise, credentialing, and education)

Responsibilities

• Provide skilled nursing care to patients in their homes.
• Perform comprehensive patient assessments and identify clinical needs.
• Develop and implement individualized plans of care.
• Monitor patient conditions and communicate changes to physicians and the care team.
• Administer medications and provide treatments as ordered.
• Educate patients and families regarding medications, disease management, safety, and self‑care.
• Complete accurate and timely clinical documentation.
• Coordinate care with physicians, therapists, aides, and other healthcare professionals.
• Participate in case conferences and care coordination activities.
• Meet the 30‑point weekly productivity expectation.
• Participate in required weeknight and weekend on‑call coverage.

Requirements

• Education: ASN or BSN
• License: Active RN license
• Certification: BLS
• Years of experience: 2
• Skills/Specialties/Technology: Home Health experience, CHHA experience, Homecare Homebase (HCHB) EMR experience
