RN - Home Health

Seeking a dedicated and compassionate Home Health Registered Nurse (RN) to provide skilled nursing care to patients in their homes throughout the assigned North Carolina service area.

Schedule & Work Details

• Coverage Areas: Robeson, Hoke, and Cumberland Counties
• Schedule: Full-time, Monday through Friday, 8:00 AM - 4:30 PM
• Weekly Hour Guarantee: 40 hours
• On-Call Requirement: Yes
• Weekend Requirement: Yes
• Mileage Reimbursement: Yes

Pay Rate: $51.00 - $53.00 per hour

Responsibilities

• Provide skilled nursing services and comprehensive patient assessments in the home while developing and implementing individualized care plans.
• Complete accurate clinical documentation and OASIS assessments while maintaining timely and compliant records.
• Provide patient and caregiver education, medication management, wound care, and other skilled nursing interventions as appropriate.
• Coordinate care with physicians and interdisciplinary team members, monitor patient progress, and support continuity of care and positive patient outcomes.

Requirements

• Active, unrestricted Registered Nurse (RN) license.
• Strong clinical assessment, communication, and organizational skills.
• Ability to work independently in a field-based setting.
• Availability to participate in required on-call and weekend coverage.

Preferred Qualifications

• Previous home health nursing experience.
• Experience using Homecare Homebase (HCHB).
• Experience managing a home health patient caseload.
• Excellent patient education and care coordination skills.
