This role is responsible for delivering skilled nursing care in the home setting, educating patients and families, managing patient treatment plans, and collaborating with physicians and the interdisciplinary care team to promote optimal patient outcomes. The ideal candidate is organized, patient-focused, and capable of working independently in a field-based environment.Schedule & Work DetailsAgency: Home Choice Health ServicesCoverage Area: Shelby CountySchedule: Monday through Friday, 8:00 AM to 4:30 PMWeekly Hour Guarantee: 32 hoursPay Rate: $51.00 - $53.00 per hourOn-Call Requirement: One weekend per monthMileage Reimbursement: Yes, reimbursed between patient visitsKey ResponsibilitiesProvide skilled nursing care and treatment in patients' homes as prescribed by the physician. Conduct patient assessments and monitor changes in health status. Develop, implement, and update individualized plans of care in collaboration with physicians and other healthcare professionals. Educate patients and family members on disease processes, treatment plans, medication management, and home care techniques. Document all patient care activities accurately and timely in accordance with agency policies and regulatory requirements. Report changes in patient condition to the supervising nurse and physician as appropriate. Manage and organize patient care supplies, equipment, and medications. Coordinate care with physicians, therapists, aides, and other members of the interdisciplinary team. Promote patient safety, independence, and quality of life through evidence-based nursing interventions. Maintain compliance with agency, state, federal, and payer requirements. Participate in on-call coverage as assigned. Required QualificationsActive Registered Nurse (RN) license in good standing. Previous nursing experience required. Strong clinical assessment and patient care skills. Valid driver's license, reliable transportation, and proof of automobile insurance. Ability to work independently and manage a field-based caseload. Excellent verbal and written communication skills. Strong documentation and organizational abilities. Preferred QualificationsPrevious home health experience. Excellent organizational and time management skills. Strong problem-solving and critical thinking abilities. Experience coordinating care across multidisciplinary teams. Ability to build positive relationships with patients, families, and referral sources. Dress CodeScrubs required. No specific scrub color preference.
