About the Role
Seeking a compassionate and experienced Home Health Registered Nurse (RN) to provide skilled nursing care to patients throughout Collier County. This role is ideal for a nurse who enjoys working independently while making a meaningful impact on patients' health and quality of life in the home setting.
Schedule & Pay
Coverage Area: Collier County, FL
Schedule: Full-time, 40 hours per week
Shift: Monday through Friday, 8:00 AM - 5:00 PM
On-Call/Weekend Requirement: As needed
Mileage Reimbursement: Yes
Patient Care Setting: Patient homes throughout the service area
Pay Rate: $51.00 - $53.00 per hour
Responsibilities
- Provide skilled nursing visits to patients in their homes in accordance with physician orders and individualized care plans.
- Complete OASIS assessments, including:
- Start of Care (SOC)
- Resumption of Care (ROC)
- Recertification Visits
- Discharge Assessments
- Perform comprehensive head-to-toe assessments and ongoing evaluations of patient status.
- Deliver skilled nursing interventions including wound care, ostomy care, Foley catheter care, and other clinical treatments as ordered.
- Monitor patient progress and identify changes in condition requiring physician notification or care plan modification.
- Educate patients and caregivers regarding disease management, medications, treatment plans, and home safety.
- Coordinate care with physicians, therapists, aides, and other members of the healthcare team.
- Maintain timely, accurate, and compliant documentation within the electronic medical record system.
- Ensure adherence to Medicare, state, and agency regulations and standards.
- Participate in case conferences and interdisciplinary care planning activities.
- Manage a daily patient schedule while traveling throughout the assigned coverage area.
Requirements
- Active Registered Nurse (RN) license in the State of Florida.
- Ability to perform comprehensive assessments and skilled nursing visits independently.
- Valid driver’s license, reliable transportation, and ability to travel throughout Collier County.
- Strong clinical assessment, organizational, and time-management skills.
- Excellent verbal and written communication skills.
Preferred Qualifications
- Previous home health experience.
- Experience completing OASIS assessments and documentation.
- Knowledge of Medicare home health regulations and compliance requirements.
- Strong patient education and care coordination abilities.
- Excellent communication skills and professional bedside manner.
- Ability to work independently and effectively manage a field-based caseload.
