About the Role
Seeking an experienced and compassionate Registered Nurse (RN) to provide skilled home health services to patients throughout the Aberdeen service area. This role requires strong clinical judgment, exceptional organizational skills, and the ability to work independently while coordinating care across the healthcare continuum.
Schedule
- Weekly Hour Guarantee: 40 hours
- Coverage Area: 60-mile radius from Aberdeen
- Schedule: Days, Monday through Friday
- Shift: 8:00 AM - 4:30 PM
- On-Call/Weekend Requirement: Yes
- Mileage Reimbursement: Yes, between patient visits
Pay
$51.00 - $53.00 per hour
Responsibilities
- Complete Start of Care (SOC) assessments and establish individualized plans of care.
- Perform OASIS documentation accurately and in compliance with agency and regulatory requirements.
- Conduct Recertification (Recert) visits and reassess patient needs throughout the episode of care.
- Complete Resumption of Care (ROC) assessments following hospitalizations or care interruptions.
- Perform comprehensive nursing evaluations and ongoing skilled nursing visits.
- Provide skilled nursing interventions including medication management, disease process education, wound care, and treatment administration.
- Monitor patient progress and communicate changes in condition to physicians and other members of the care team.
- Develop, update, and coordinate patient care plans to promote optimal outcomes.
- Educate patients and caregivers regarding diagnoses, medications, treatments, home safety, and self-management strategies.
- Coordinate care with physicians, therapists, social workers, home health aides, and other interdisciplinary team members.
- Maintain timely, accurate, and compliant clinical documentation.
- Participate in on-call and weekend coverage as assigned.
- Ensure compliance with Medicare regulations, agency policies, and professional nursing standards.
Requirements
- Active and unrestricted Registered Nurse (RN) license.
- Current CPR certification.
- Valid driver's license.
- Reliable transportation and ability to travel throughout the assigned coverage area.
- Strong clinical assessment and critical thinking skills.
- Ability to work independently in a field-based environment.
- Excellent communication, organizational, and documentation abilities.
Preferred Qualifications
- Previous home health experience.
- Experience using Homecare Homebase (HCHB).
- Wound care experience.
- Knowledge of OASIS documentation and Medicare home health regulations.
- Experience managing an independent patient caseload.
- Strong patient education and care coordination skills.
- Ability to build positive relationships with patients, families, and healthcare providers.
