About the role

The Registered Nurse completes initial and periodic comprehensive assessments of clients, caregivers, and home environments for admission to and continued service from the Agency. This includes determining needs, evaluating the effectiveness of care provided, and assessing achievement of anticipated outcomes. Assessments may include OASIS items at appropriate time points.

Responsibilities

• Accepts client admissions as assigned by the Agency and weekend on-call assignments.
• Provides comprehensive nursing care through home assessments, nursing diagnoses, and physician-approved care plans.
• Performs treatments requiring specialized skills as ordered by the physician.
• Observes signs and symptoms and reports them to the VP of Clinical Services, Director of Professional Services, and/or physician as appropriate.
• Notes changes in the physical or emotional status of clients.
• Assesses nutritional needs and diet of clients as ordered by the physician.
• Educates patients and families on medication and procedures.

Requirements

• Holds a current Registered Nurse license.
• Graduate from an approved school of nursing.
• Minimum of two years of experience in an acute setting requiring heart, lung, and physical assessments, or demonstrated ability/education in these areas.
• Current CPR certification.
• Excellent communication, organizational, and human relations skills.
• Able to function with minimal supervision.
• Able to manage multiple projects and changing priorities.
• Employs critical thinking skills.
