Responsibilities

• Perform comprehensive nursing assessments and develop individualized plans of care
• Provide skilled nursing care, patient education, and care coordination in the home
• Collaborate with physicians, therapists, medical social workers, and other members of the care team
• Monitor patient progress and communicate changes in condition promptly
• Complete accurate and timely documentation while maintaining Medicare and agency standards

Requirements

• Current Florida Registered Nurse (RN) license
• Valid driver’s license and reliable transportation
• Excellent clinical, communication, and organizational skills
• Home health experience required
• OASIS experience required
