About the role

We have a great opportunity for a Home Health RN! This is a full-time position supporting Forsyth County and surrounding areas. CPR certification from the American Heart Association or American Red Cross Basic Life Support (BLS) is required. We are seeking a dynamic, quality-driven Registered Nurse (RN) to pave the way for outstanding care delivery for our population of home health patients. The ideal candidate will have a passion for providing exceptional, compassionate care to our home health patients, ensuring high-quality outcomes and improving the overall patient experience. As a Home Health RN, you will be responsible for delivering patient care, coordinating with interdisciplinary teams, and driving continuous improvements in care quality within our home health program. Requirements

• CPR certification from the American Heart Association or American Red Cross Basic Life Support (BLS)
• Current, active Registered Nurse (RN) license in North Carolina
• Bachelor’s degree in Nursing (BSN) preferred
• Minimum of 2 years of experience in nursing, with at least 1 year in a home health or community health setting preferred
• Experience in leading or coordinating patient care strongly preferred
• Strong clinical assessment and critical thinking skills
• Excellent leadership, communication, and interpersonal skills
• Ability to work independently while managing a caseload of home health patients
• Knowledge of home health care regulations, including Medicare and Medicaid guidelines
• Proficiency with electronic health records (EHR) and other healthcare software
• Strong organizational and time management skills, with the ability to prioritize patient needs
• Valid state-issued driver's license required
• Must carry automobile liability insurance at limits required by agency
• Must have own transportation
