RN - Home Care Nurse

About the role

We are seeking a Registered Nurse (RN) with a passion for adult care to join our Skilled Nursing team. You will provide one-to-one care for a 28-year-old client in Warminster with a history of cerebral palsy, congenital hydrocephalus, neuromuscular scoliosis (status post spinal fusion), intrathecal baclofen pump placement, gastrojejunostomy (G-J) tube dependence for enteral nutrition, and gastroesophageal reflux disease (GERD). The client enjoys listening to music, dancing by shaking his hands, being read to, and spending time outdoors in his wheelchair.

Available shifts include day, evening, and overnight.

Responsibilities

• Travel to the client’s home on a schedule you choose
• Follow up with, execute, and properly document doctors' orders
• Perform assessments and monitor the client’s condition
• Document observations, interventions, and evaluations
• Provide hands-on care with autonomy in the home
• Build client relationships and celebrate milestones

• One-to-one client care (no weekend requirements)

Requirements

• Graduation from an accredited and approved nursing program
• A current nursing license in good standing in the state
