About the role

Responsibilities
- Educating patients on self-management
- Supporting patients, families, and communities
- Collaborating with team members to deliver quality, patient-centered care
Qualifications
- Minimum of 1 year med-surg experience (preferred home care experience)
- BSN preferred
- IV and venipuncture skills
- Proficiency with technology
- Ability to travel within the service area
- Licensure in the state
- Valid driver's license
- Current CPR certification

- Strong clinical competencies
- Effective communication
- Ability to work independently
Schedule
May involve working weekends, holidays, and on-call hours.
