About You

We are seeking passionate, committed professionals who want to make a difference in the lives of patients and families in our community.

• Registered Nurses (RN) licensed in the state of employment.
• Licensed Practical/Vocational Nurses (LPN/LVN).
• Home Health Aides (HHA) or Certified Nurse Aides (CNA) with a heart for compassionate care.
• Professionals committed to providing patient-focused care and building strong relationships with patients, families, and referral sources.

Pay

Compensation may vary within the salary range provided based on several factors, including but not limited to a candidate's geographic location, experience, education, skills, licensure, certifications, internal equity, and department equity.

• Register Nurse Case Manager Salary Range: $95,000.00 - $105,000.00
• After Hours Care Manager RN Salary Range: $105,000.00 - $115,000.00
