Wage Range: $45.19 – $82.30 per hour. Posted range reflects the minimum of the lowest level and the maximum of the highest level. Additional premiums may be offered based on shift, certifications, or degrees. Offers are determined based on experience, education, and internal equity.
Job Summary
EvergreenHealth’s Heart Care Clinic is seeking an experienced Registered Nurse to provide nursing care through face‑to‑face or telephone interactions. The RN will analyze patient‑provided information to determine appropriate care, deliver patient education, ensure care meets established standards, and may assist in developing case‑management protocols.
Primary Duties
- Manage clinic flow and ensure continuity of care across clinic, hospital, ER, specialists, Home Health, Hospice, and LTC facilities.
- Triage patients in person and via phone.
- Assess comprehensive nursing needs and develop nursing diagnoses.
- Collaborate with providers to develop and implement plans of care.
- Analyze diagnostic and lab test results and relay information to providers.
- Coordinate critical results in a timely manner.
- Oversee medical assistants to ensure care is delivered within guidelines and evaluate outcomes.
- Assist with the Medical Home pilot by attending collaborative meetings, preparing month‑end reports, and working with the nurse case manager on patient case management.
- Perform nursing visits within scope of practice and help develop group visits with managers and providers.
- Provide education and training for medical assistants in collaboration with the manager and/or RN Quality Manager.
- Balance workloads and adjust staffing across pods to adequately cover providers.
- Deliver safe, family‑centered nursing care and serve as a consultant to patients and caregivers.
- Perform other duties as assigned.
Requirements
Required:
- Graduate of a nursing program (Diploma, ADN, BSN, or MSN).
- Licensed Registered Nurse in Washington State or holding a multistate Nurse Licensure Compact (NLC) license.
- Minimum 3 years of RN experience.
- Experience in chronic care management and patient education.
- Ability to manage multiple concurrent programs and projects effectively.
- Current Healthcare Provider BLS certification (American Heart Association) by start date.
- Current Healthcare Provider ACLS certification by date of hire.
Desired:
- Previous lead or supervisory experience with effective feedback skills.
- Master’s in Nursing (MSN).
- Recent ICU/CCU/PCU/Cath Lab experience.
