About the Role

WellMed, part of the Optum family of businesses, is seeking a Registered Nurse (RN) to join our team in San Antonio, TX. As a member of the Optum Care Delivery team, you'll be an integral part of our vision to make healthcare better for everyone. This role supports patients with high-risk health conditions, helping them navigate the healthcare system while acting as an educator, resource, and advocate.

The Healthcare Manager works in a less structured, self-directed environment and performs all nursing duties within the scope of an RN license. You'll collaborate with multidisciplinary care teams, including physicians, nurses, pharmacists, and social workers, to ensure a maximum quality of life for patients and their families.

Responsibilities

  • Work with providers and clinic staff to identify high-risk patients.
  • Support longitudinal care of patients with chronic conditions by:
    • Performing assessments of health conditions.
    • Conducting medication reconciliation.
    • Using motivational interviewing and self-management goal setting.
    • Providing patient education.
    • Creating referrals to appropriate agencies and resources.
  • Support transitions of patients with chronic conditions from inpatient to outpatient settings by:
    • Performing assessments of transitional needs.
    • Conducting medication reconciliation.
    • Establishing and reviewing contingency plans.
    • Providing patient education.
    • Assisting with post-discharge needs such as prescriptions, transmutation, Durable Medical Equipment (DME), and appointments.
  • Coordinate with providers to establish or update plans of care.
  • Perform accurate and timely documentation in the electronic medical record.
  • Participate in daily huddles and Patient Care Coordination (PCC) meetings.
  • Prepare accurate and timely reports for weekly meetings.
  • Maintain continued competence in nursing practice and knowledge of current evidence-based practices.
  • Perform clinical tasks within the scope of practice.
  • Perform all other related duties as assigned.

Requirements

  • Bachelor's degree in Nursing or Associate's degree in Nursing with 4+ years of experience.
  • Registered Nurse with an active and unrestricted license to practice in the state of employment.
  • Current BLS certification.
  • 2+ years of experience in a physician's office, clinical, or hospital setting.
  • Experience interacting productively with individuals and multidisciplinary teams.
  • Knowledge of chronic diseases, especially COPD/asthma, diabetes, CHF, and IHD.
  • Proficient computer skills to work efficiently with electronic medical records.

Preferred Qualifications

  • Experience related to patient education, motivational interviewing, and self-management goal setting.
  • Proven excellent verbal and written communication skills.
  • Strong interpersonal skills.
  • Excellent organizational and prioritization skills.
  • Bilingual.

Pay

The salary for this role ranges from $60,200 to $107,400 annually based on full-time employment, depending on factors such as local labor markets, education, work experience, and certifications. We comply with all minimum wage laws as applicable.
