About The Role

As a Partner in Wellbeing, the RN Health Coach provides 'whole person' care to address acute and chronic medical conditions, compiling and evaluating medical histories and their impact on behavioral health diagnoses.

Essential Responsibilities

• Completes client screening and assessment related to overall physical health and the impact on the management of behavioral health concerns.
• Provides short-term medical case management for acute client conditions.
• Collaborates with other Alluma providers to ensure the safe delivery and monitoring of medication plans.
• Provides direct care services as a community provider.
• Collaborates with other community providers.
• Ensures that physical health care coordination is included in the person-centred plan, through collaboration with the client’s primary care provider and other specialty services.
• Fulfills roles in integrated health care (physical, mental, substance use, and oral), ensuring referrals to necessary internal and external services.
• Consults, teaches, and individualizes treatment plans for specific cases and problems.
• Affords assistance with whole health care, including prevention/screening/follow-up.
• Assists with withdrawal management, coordinating with the medication provider.
• Collaborates with the care coordination team to identify clients admitted/discharged from inpatient hospitalization for smooth community integration.

Qualifications

• Current MN and ND Registered Nurse License.
• Two plus years of experience in direct nursing, either clinic or community-based; psychiatric experience, preferred.
• CPR certified.
• Must pass a criminal background check through MN DHS NetStudy.
• Access to a personal vehicle and the ability and willingness to use the vehicle in carrying out position responsibilities.
• Valid driver’s license and insurable under the agency’s automobile insurance policy.
• Free from substance use problems for no less than 2 years.
