Core Job Responsibilities
- Support the PCP in the delivery of care by gathering and documenting relevant clinical data, as appropriate and per patient need.
- Assists in specialty office procedures as directed.
- Participate in the daily huddle to identify individual patient needs for upcoming appointments.
- Communicate effectively with other members of the care team in coordinating the individualized plan of care.
- Support PCP during in office procedures and examinations.
- Provide outreach and education to patients in need of services as identified through quality improvement initiatives (Population Health Management).
- Perform duties within scope of practice to the highest level of licensure.
- Provide direct support & guidance to other members of the care team as appropriate.
- Able to perform assessment data collection on all patients utilizing tools / forms in EMR.
- Adequately assess and reassess pain. Educate the patient and family regarding pain management.
- Provide age appropriate care to patients in the community setting.
- Consistently document procedures completed in the EMR, to include medications administered and follow-up assessments.
- Deliver medication utilizing the five (5) rights and observes patient for desired untoward effects.
- Maintain current knowledge of medications and their correct administration.
- Establish priorities of patient care based on needs and office resources.
- Demonstrates the ability to assist providers with procedures and perform services requiring technical and manual skill.
- Perform other duties as required.
- Graduate of an accredited school of nursing or nursing program.
- Current New York State Registered Nursing registration.
- BLS Certification.
- Minimum 3 years of nursing experience
- Basic computer and EMR knowledge.
- Excellent communication and interpersonal skills.
- Able to assimilate information from many different sources.
- Must be able to communicate in English both verbally and in writing.
- Required: Current New York State licensure as a Registered Nurse.
- Prior to start date, must obtain BLS certification.
