RN Field Clinical Care Coordinator - Saugus, Melrose, Malden, MA

About the role

We are seeking a passionate RN Clinical Care Coordinator to serve as the primary care manager for members with complex medical and behavioral needs.

Responsibilities

• Conduct comprehensive needs assessments face-to-face or telephonically, evaluating medical, behavioral, functional, cultural, and socioeconomic factors.
• Develop and implement person-centered care plans focused on managing chronic conditions, health promotion, social determinants, medication safety, and member safety, aligning with evidence-based guidelines.
• Collaborate with care teams, providers, and community resources to execute care plans effectively.
• Provide education and coaching to support member self-management and lifestyle modifications.
• Assist with discharge planning and care transitions following ER visits, hospital stays, or SNF admissions.
• Advocate for members to ensure their needs and preferences are prioritized within the healthcare team.

Qualifications and skills

• High School Diploma/GED and current, unrestricted RN licensure in MA.
• At least 2 years of clinical experience.
• Proficiency with MS Office and access to a secure home workspace.
• Reliable internet, transportation, and a valid US driver's license for local travel.
• Preferred: Bachelor's/Master's in Nursing, CCM certification, community case management experience, and familiarity with electronic health records and virtual care platforms.

Pay and schedule

• This role involves up to 75% local travel, working remotely from a home-based office, with mileage reimbursement.
