About the role
The Diabetes Services Educator provides patient education in outpatient and community settings under the leadership of the Diabetes Services Program Manager.
Responsibilities
- Participates in multi-disciplinary rounds
- Interacts empathetically and compassionately with patients and their families regarding medical needs
- Articulates medical science related to diabetes and translates knowledge into layman's terms
- Facilitates and teaches individuals and groups in one-to-one counseling sessions, group classes, public speaking engagements, and through print, audio, and video media
- Possesses a broad scope of knowledge and skills to provide person-centered education for self-care management
Requirements
- At least 3 years of broad clinical nursing experience
- A focus in diabetes education with 2 years experience as a Diabetes Educator working with patients is preferred
- Formal and informal teaching experience to include facilitating and teaching individuals and groups is preferred
- Spanish and other foreign language fluency is preferred
- The ability to convey a positive, warm, friendly, and healing presence to patients and the community with excellent customer service is required
Qualifications
- Bachelor’s degree in nursing strongly preferred
- State of California RN license required
- Certified Diabetes Care and Education Specialist (CDCES) certification preferred upon hire but required within 24 months, or at the completion of 1000 hours of providing patient diabetes education
- American Heart Association Healthcare Provider BLS certification required
