RN - Clinical Transition Specialist

The role involves overseeing and managing patient care during hospitalization, ensuring a safe and timely transition from acute care to appropriate post-hospital services. Key responsibilities include conducting assessments, coordinating care, monitoring progress, and evaluating outcomes using multidisciplinary team collaborations and daily bedside rounds. Responsibilities also encompass acting as a liaison with patients, families, and physicians to determine next care steps, managing discharge planning, documenting care plans, and initiating interventions across pre-hospital, hospital, and post-hospital phases. Qualifications include active licensure as an RN, Basic Life Support certification, and accreditation as a Case Manager.
