RN Clinical Team Specialist | Homecare Home Base | OASIS

As a Clinical Team Specialist, you’ll provide oversight of all patient care services, ensuring high-quality care to more than 60,000 patients daily in their preferred home setting. You’ll play a key role in coordinating care, reviewing plans, and maintaining compliance with state and federal regulations while contributing to the organization’s strategic and quality improvement initiatives.

Responsibilities

• Provides oversight of all patient care services, including coordinating patient care, referrals, and ensuring patient needs are continually assessed.
• Develops, implements, and updates individualized patient plans of care.
• Reviews and approves plans of care, evaluating proposed changes for clinical appropriateness.
• Conducts regular OASIS reviews for all patients.
• Ensures required visits are completed in a timely manner.
• Reviews plans of care and documentation for accuracy and compliance with State and Federal requirements.
• Monitors timeliness of required documentation uploaded to HCHB by direct care team members and LPN/LVN Clinical Coordinators.
• Maintains records and documentation, ensuring accuracy, completeness, and compliance with licensing regulations, certification standards, and legal/ethical imperatives.
• Participates in the organization’s strategic planning and QAPI (Quality Assurance and Performance Improvement) activities.
• Performs other duties and projects as assigned, in a physically demanding, high-stress environment.
• May be asked to travel occasionally to support a field office during audits or necessitating events.

Requirements

• Associate degree in a Nursing-related field (required).
• 2 years of home care experience as a registered nurse in a Home Health or Hospice environment.
• Current, unrestricted RN license valid for the state of work.
• Ability to quickly become proficient in enterprise applications such as Homecare Homebase, Workday, and Microsoft Office.
• Must possess a valid driver’s license, maintain an acceptable driving record, or have reliable transportation to travel between patient homes and assigned work locations.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
