RN Clinical Supervisor - SIGN ON INCENTIVE

About the Role

Oversee all patient care and clinical personnel, ensuring care is coordinated, compliant, timely, and aligned with individualized plans of care and agency standards.

Responsibilities

• Supervise all patient care services and staff (RN, LPN, OT, PT, HHA)
• Coordinate patient care and referrals
• Ensure ongoing assessment of patient needs
• Provide direct patient care when staffing requires
• Support clinicians with OASIS, documentation and case management
• Review clinical documentation
• Lead case conference, clinical meetings, and in-services
• Ensure patient charts and records are accurate and regulatory compliant
• Monitor clinical outcomes

Requirements

• Registered Nurse (RN) with current state license
• 2-3 years of home health experience required
• Strong leadership, communication, and problem-solving skills
• CPR Certification
• Valid driver’s license and auto insurance

Benefits

• Sign-on bonus: $1,250 after the first 90 days, and another $1,250 after one year of employment
