RN Clinical Coordinator, Heart and Vascular

About the role

This position is responsible for the programmatic development of assigned program(s), including care coordination and outreach management. The role involves clinical management of specialized multidisciplinary care pathways, organized communication between care continuum stakeholders, research, education, compliance with regulatory requirements, and implementation of annual strategic objectives.

Responsibilities

• 50% Care Coordination:
  • Serve as a primary contact for clinical expertise related to the assigned programs.
  • Actively participate in all phases of program care pathway development.
  • Cultivate, implement, and monitor evidence-based standards of care for program patients.
  • Establish and attain challenging clinical, operational, and quality performance goals.
  • Coordinate with interdisciplinary teams to advance patient care.
  • Ensure appropriate post-discharge education with patients and families.
  • Evaluate the quality and effectiveness of patient care and support services; analyze and ensure corrective action as needed.
  • Perform initial, ongoing, and functional health status assessments for the population and/or individuals.
  • Collect, analyze, and document subjective and objective patient data.
  • Review data obtained by other healthcare team members and incorporate it into assessments.
  • Identify and communicate assessment findings through referrals, rounds, huddles, reports, and care conferences.
  • Use assessment data to identify nursing diagnoses and collaborative problems.
  • Collaborate with providers and healthcare team members on assessment findings.
  • Document planned nursing interventions to achieve outcomes tailored to patient needs.
  • Initiate and/or coordinate care plans and individualize critical paths/medical management plans.
• 20% Outreach Management:
  • Facilitate communication between referring hospitals, providers, and EMS regarding patient information, training, and education.
  • Provide community education on assigned programs.
  • Act as a liaison between patients, departments, and relevant OhioHealth service lines at the system level.
• 20% Deployment and Maintenance of Program Imperatives:
  • Collaborate with departments to ensure real-time readiness and compliance with regulatory, accreditation, and licensing standards.
  • Maintain knowledge of current Joint Commission guidelines and objectives.
  • Assist in developing and implementing annual strategic initiatives.
  • Collaborate with stakeholders to improve program value, including initiatives to lower the total cost of care.
  • Participate in process improvement activities.
• 5% Advancement of Program at the Local and National Level:
  • Participate in forums to exchange knowledge and information related to program advancement.
  • Maintain competency in best practices and emerging trends.
• 5% Clinical Care:
  • Participate in some aspects of clinical care as needed, both in-person and virtually.

Requirements

• Bachelor’s Degree (required).
• Current Ohio licensure as an RN.
• Experience in clinical program development and operations.
• A minimum of three years of experience in assigned program(s).
• Ability to work in a complex clinical and/or administrative system or program.
• Strong communication skills to lead, teach, and influence others.
• A high level of analytical ability.

Specialized knowledge in nursing or a related healthcare field (completed or in progress).
