RN Clinical Assessor (Kanawha or Boone Counties)

About the role

Acentra Health is seeking a RN Clinical Assessor (Aged & Disabled Waiver) to join our dynamic team. This role involves conducting medical eligibility assessments, managing caseloads, and ensuring timely data entry.

Responsibilities

• Document contacts, schedule appointments, and travel to conduct Pre-Admission Screenings
• Participate in inter-rater reliability activities and denial hearings
• Contact physicians and facilities to confirm medical information
• Conduct assessments to determine ADW eligibility
• Maintain privacy and dignity during assessments
• Conduct service plan reviews and submit assessments using state-approved interfaces
• Respond to state inquiries and participate in mediation and appeal processes
• Attend staff meetings and conduct case consultations/peer reviews
• Provide assessments for initial eligibility determinations and annual assessments
• Accurately assess medical conditions and symptoms

Qualifications

• Valid, unrestricted RN license for West Virginia or compact license
• Valid driver's license and reliable transportation
• Willingness to travel within and outside catchment areas
• Excellent organizational and customer service skills
• Ability to assess medical conditions and functional abilities
• Computer skills, including Microsoft Outlook, Word, and Google Chrome
• Two years of experience in a home care setting or with the aging population/disabled
• Knowledge of Aged and Disabled Waiver program policies and procedures
