RN Clinical Assessor (Kanawha or Boone Counties)

About the role

Acentra Health is seeking a RN Clinical Assessor (Aged & Disabled Waiver) to join our dynamic team. This role involves educating and performing medical eligibility assessments for WV Medicaid recipients seeking community-based services.

Responsibilities

• Document contacts, schedule appointments, and travel to conduct Pre-Admission Screenings within prescribed timelines
• Participate in inter-rater reliability activities and denial hearings as necessary
• Contact physicians and facilities to confirm medical information
• Conduct assessments to determine ADW eligibility using state-approved standardized tools
• Maintain privacy and dignity during assessments
• Conduct service plan reviews as needed
• Submit assessments using state-approved interfaces
• Respond to state inquiries regarding assessments
• Attend and participate in staff meetings and case consultations
• Provide guidance and assistance to beneficiaries
• Conduct initial and annual assessments for 1915(c) HCBS programs
• Read, understand, and adhere to all corporate policies, including HIPAA

Qualifications

• Valid, unrestricted RN license for West Virginia or compact license
• Valid driver's license and access to reliable transportation
• Willingness to travel within and outside the catchment area
• Excellent organizational and customer service skills
• Good interpersonal communication and flexibility
• Ability to assess medical conditions and functional abilities
• Computer skills, specifically with Microsoft Outlook, Word, Edge, and Chrome

Preferred Qualifications

• Two years of experience in a home care setting
• Two years of experience with the aging population or persons with disabilities (preferably within the WV Aged and Disabled Waiver Program)
• Knowledge of Aged and Disabled Waiver program policies and procedures
