About the role

Responsibilities

• Assesses present and potential needs of patient and family
• Reviews Plan of Care with patient, family, physician, IDT and review/evaluate/modify routinely
• Establish realistic goals of care to ensure continuity and quality of care
• Completes all documentation in a concise, timely manner per agency standards
• Manages symptoms and teaches patient/family care management
• Be at work and be on time
• Follow company policies, procedures and directives
• Interact in a positive and constructive manner
• Prioritize and multitask

Requirements

• Minimum 3 year’s relevant experience as a Registered Nurse in an acute care setting
• Home Health, Oncology, and/or Hospice experience preferred
• Effective problem-solving skills
• Good interpersonal and communication skills
• Able to relate to a variety of health professionals in different settings
• EMR experience

Qualifications

• Current RN license issued by the California Board of Registered Nursing
• BSN preferred
• BLS issued by the American Heart Association
• Hospice and Palliative Care Certification preferred
