About the role
Join a dynamic team dedicated to improving patient outcomes through chronic care management. We are seeking an experienced RN Chronic Care Case Manager to lead the care of complex patients.
Responsibilities
- Develop and implement care plans tailored to individual patient needs
- Coordinate care among various healthcare providers and services
- Monitor patient progress and adjust care plans as necessary
- Provide education and support to patients and their families
- Document all interactions and care provided in accordance with regulatory requirements
Requirements
- Bachelor’s degree in Nursing or related field
- Current RN license in good standing
- At least 2 years of acute care nursing experience
- Strong clinical judgment and problem-solving skills
- Excellent communication and interpersonal skills
Qualifications
- Experience with chronic disease management
- Knowledge of relevant healthcare policies and regulations
- Proficient in electronic health record systems
Skills
- Strong organizational and time management skills
- Ability to work independently and as part of a team
- Effective written and verbal communication skills
