Make a Lasting Difference as a Hospice Registered Nurse Case Manager (RNCM)! Hearth Hospice is seeking a Full Time Hospice RN Case Manager (RNCM) to join our dedicated nursing team providing exceptional end-of-life care throughout the community.

Monday - Friday, 8 am - 5 pm

Responsibilities

• Respond to clinical referral information with care and expertise, addressing inquiries and requests with a compassionate touch.
• Partner closely with the Clinical Director to assess the eligibility and suitability of clients for specialized hospice services.
• Deliver direct nursing care as an RN Case Manager, ensuring patient comfort and quality of life.
• Apply nursing skills to deliver care rooted in proven principles and techniques, easing pain and bringing solace to patients.
• Empower patients and families through education, encouraging active participation in personalized care plans aligned with their goals.
• Share responsibility for call duties, demonstrating commitment to being present when it matters most.

Qualifications

• Hold current unencumbered license as a Registered Nurse.
• Minimum one (1) year experience as an RN in a medical surgical/acute care setting.
• Hospice experience is a plus.
• Computer proficiency in typing and various programs, including experience with EMR.
• Possess and maintain current CPR certification if required by state.
• Current driver’s license, vehicle insurance, and access to dependable transportation.

Pay

$70,000 - $78,000 per year (Average Pay Range). The pay range listed represents a general posting guideline for the role and is not a fixed offer. Final compensation will be determined based on the candidate’s relevant experience, qualifications, and the specific responsibilities of the position. The exact compensation rate will be discussed and confirmed at the conclusion of the interview process.
