The Case Manager assists with coordination of services through the continuum of care and in balancing quality patient care with cost-effective utilization of resources.

Responsibilities

• Coordinates admission, treatment, and discharge plans and ensures proper utilization of hospital services
• Ensure care is provided in the most appropriate level for meeting patient care needs
• Serves as a liaison to ensure appropriate reimbursement for hospital stays and services
