RN Case Manager - PRN

About the Role

This position provides PRN care management support, advocating for patient health, safety, and self-care.

Responsibilities

• Develops individualized care plans addressing barriers to care.
• Manages complex medical cases and facilitates safe patient transitions across care settings.
• Utilizes telehealth and home-based technologies to support patient care.
• Collaborates with interdisciplinary teams to align care with patient values and goals.
• Engages patients and families in care planning and decision-making.
• Ensures care continuity, addresses gaps, and leverages community resources.
• Maintains confidentiality and documents activities promptly.
• Works independently within legal and ethical guidelines.

Requirements

• Licensed Registered Nurse (RN).
• Bachelor’s degree or higher in nursing or related field.
• CCM or RN-BC certification preferred or attainable within 36 months.
• Proficiency with Microsoft Office and knowledge of managed care principles.
• Strong critical thinking and assessment skills, particularly for cardiovascular and respiratory conditions.

Working Conditions
