About the Role
This position provides PRN care management support, advocating for patient health, safety, and self-care.
Responsibilities
- Develops individualized care plans addressing barriers to care.
- Manages complex medical cases and facilitates safe patient transitions across care settings.
- Utilizes telehealth and home-based technologies to support patient care.
- Collaborates with interdisciplinary teams to align care with patient values and goals.
- Engages patients and families in care planning and decision-making.
- Ensures care continuity, addresses gaps, and leverages community resources.
- Maintains confidentiality and documents activities promptly.
- Works independently within legal and ethical guidelines.
Requirements
- Licensed Registered Nurse (RN).
- Bachelor’s degree or higher in nursing or related field.
- CCM or RN-BC certification preferred or attainable within 36 months.
- Proficiency with Microsoft Office and knowledge of managed care principles.
- Strong critical thinking and assessment skills, particularly for cardiovascular and respiratory conditions.

Working Conditions
Fast-paced, adaptable environment primarily involving telephonic and virtual visits, with potential for in-person interactions. Equipment includes standard office tools, computers, and telecommunication devices.
